Where does Remote Connected Care® provide services today?

We currently provide remote patient monitoring services to individuals in the following states: AL, AR, AZ, CT, FL, GA, ID, KY, KS, LA, MI, MN, MS, MT, NE, NV, OH, OK, OR, SC, SD, TN, UT, VA, VT, WA, WV, and WY.

How do I sign up for remote patient monitoring with Remote Connected Care®?

Which health conditions does Remote Connected Care® help support?

With our vitals monitoring devices, we’re able to provide insights and support the care of a variety of chronic health conditions. Examples of chronic conditions are hypertension, chronic obstructive pulmonary disease (COPD), and congestive heart failure (CHF).

Who do I meet with virtually at Remote Connected Care® regarding ongoing health status updates and care?

Remote Connected Care® has a variety of qualified healthcare professionals ranging from Medical Doctors, Nurse Practitioners and Registered Nurses who are dedicated to ensuring patients are educated on their conditions and questions are answered in a way that is catered to that patient’s specific health situation. These visits are completed virtually.

Does Remote Connected Care® replace my primary care doctor?

We do not replace your primary care doctor or any specialist. Instead, we work in collaboration with them to ensure you receive the attention and care you need. We will be in communication with your prescribing practitioner to ensure they are well-informed of your day-to-day status to help in your next visit.

Is it easy to use the remote patient monitoring devices?

Remote Connected Care® utilizes the best and most easy-to-use medical devices so you can easily use them from the comfort of your own home. Our Customer Service staff is always standing by should you have any questions.

How can I see my vital signs and past readings?

Log into the patient portal at https://portal.remoteconnectedcare.com/ then click on the section titled “Tracking” to see your health tracking and measurements.

How do I let my doctor know about my vital signs?

Your prescribing practitioner (doctor) will receive a monthly summary of all your vitals, and both you and your prescribing practitioner also have the option to accessing your vitals through the Remote Connected Care® portal.

What happens if I have an out of bounds or abnormal reading?

Remote Connected Care® clinical staff will decide based on your vitals history and trends whether a reading is truly irregular or abnormal, and whether a text or phone call to you, or an escalation to your physician is required. If you receive a text or phone call from our clinical staff, they will validate that your reading was accurate by asking you to retake the measurement, then they will assess next steps.

Does my insurance cover Remote Connected Care® services?

We currently support patients who have Medicare with a majority of qualified patients having little to no out-of-pocket costs for this service. As our patients are onboarded into Remote Connected Care®, they’re informed of any financial responsibility they may have.
