If your treating prescriber has determined you are a good candidate for our service, an RCC® Physician or Nurse Practitioner will virtually meet with you to learn more about your health and social history.Home Delivery and Education
Your new remote patient monitoring devices will be delivered to your home, and either set-up during a virtual visit or by a service representative, who will help you take your first readings.Ongoing Monitoring of Health Vitals
Your blood pressure, weight, oxygen saturation and pulse will be monitored by a team of clinicians who will follow the appropriate protocols in place based on different abnormal or out of bounds readings. If needed, our team will give you a call or text to make sure the out of bounds reading was accurate, then, if appropriate, recommend next steps. We will also remind you if you forget to take a reading with one of your monitoring devices.

Monthly Follow-up Appointments
In addition to day-to-day reviews of your vitals data, you will have a dedicated Clinician who will virtually meet with you on a monthly basis to conduct a general checkup, to answer questions, and to provide you an update of where you are regarding your vitals readings.Contact Remote Connected Care®
